A Clinically Tested Probiotic Formula Although associated with spoiled food and infections, not all bacteria are harmful. As humans, we carry more bacterial cells than human cells. Most of these bacteria are found in our gastrointestinal tract where billions of bacteria reside in a delicate equilibrium that is essential to our health. This mutually beneficial relationship between enterobacteria and the human body is important for the maturation of the immune system, for a normal inflammatory response and for a healthy gastrointestinal tract. The intestinal flora is also essential for digestion and produces several important nutrients from otherwise indigestible foodstuffs.
Healthy gut bacteria act as a barrier by preventing the attachment of harmful bacteria to the intestinal mucosa. Disturbances in this delicate gastrointestinal microflora increase the susceptibility to pathogens and raise the risk of infection and disease. Probiotics help to reestablish and support a normal bacterial microflora in the intestines.
ProBiotic 3 is a probiotic formula that has been used clinically for more than 50 years in Japan. ProBiotic 3 contains three bacterial species with specific health promoting attributes:
Streptococcus faecalis S. faecalis is a natural resident of the human gastrointestinal tract that produces lactic acid as a byproduct of carbohydrate fermentation. Lactic acid reduces the gastrointestinal pH thus preventing the growth of harmful bacteria.
Clostridium butyricum C. butyricum is also a natural resident of the human gastrointestinal tract and breaks down dietary fiber into several beneficial nutrients one of which is butyric acid. Butyric acid is an important source of nourishment for the colonocytes (the cells of the lower gastrointestinal tract) and reduces inflammation and intestinal permeability.
Bacillus mesentericus B. mesentericus supports the growth of S. faecalis and C. butyricum as well as the growth of several strains of the beneficial Bifidobacterium species.
Flora in the GI Tract The gastrointestinal tract (GIT) of an adult person is approximately a 6.5m convoluted tube that is divided into the upper portion consisting of the oral cavity, esophagus and the stomach and the lower portion which consists of the small and large intestines. Because of the multiple folds and finger-like projections – microvilli – the surface area is approximately 200 times that of the skin!
The GIT houses several thousands of diverse and complex microbial population, only a fraction of which have been identified and studied. The lower portion of the human GIT is a more densely populated ecosystem than the planet, making the GIT a key organ in the body.
Each organism is classified into different genus, species and strains, producing thousands of similar yet separate strains performing distinct functions. There are three types of organisms:
• Commensals -which are neither good or bad
• Symbionts -which are beneficial to health
• Pathobionts -which cause toxicity and disease
However, all the symbionts can be classified into the following categories:
• Lactobacillus sps.
• Others which include yeast, E. coli, propinobacterium, etc.
The term ‘probiotics' was first used in 1953 to indicate the health-providing benefits in contrast to the harmful antibiotics which kill organisms. The WHO definition is “live organisms which, when administered in adequate amounts, confer health benefits to the host.” Some of the health benefits are:
• Act as a barrier against pathogens e.g. that cause diarrhea
• Generate various nutrients like vitamins B2, B12, K, etc.
• Helps absorption of important minerals like calcium
• Help produce important digestive enzymes
• Cholesterol-reducing effect
• Stimulates the immune system
• Enhances bowel motility and relieves constipation
• Maintains mucosal integrity
• Reduces inflammation and allergic reactions
• Helps detoxification
• Prevents colonization of pathogenic bacteria by competing for space
Probiotics have been used for centuries, but only recently (the last 100 years) have they been studied in depth. Japan is the world leader in the use of probiotics in various types of foods like yogurt, candy, milk, desserts, etc.
For any probiotic, the following are requirements:
• Clinically studied. The more studies, the better (as there is a strong placebo effect to contend with). Products not subject to a clinical study need to be viewed with caution. Do not take the word of any manufacturer! Ask for proof.
• The right species that are human strains.
• Resistant to mutation, i.e. not subject to genetic change so that these beneficial organisms don't become toxic themselves or help spread antibiotic resistance, which is a real concern!
• Must be stable and resistant to stomach acid, bile salts and digestive enzymes. It also helps if the organisms are room stable for convenience!
• Ability to adhere to the intestines and colonize. If the organisms cannot set up house in the crowded GIT then they are pretty much useless and pass through the GIT every time. There is an exception, and that is the yeast strains, but not the bacteria.
• Documented ability to deliver health benefits. E.g. stimulate the immune system, have anti-allergic effect, detoxify, etc.
• Must be compatible with each other and not only that but be synergistic. Most products have a hodge-podge of strains thrown together with no idea if they are compatible except a hope that they will work together and not kill each other. Ask for proof! Clinical studies are the best proof!
In a study that evaluated the effectiveness of Bio-three in benefiting patients with ulcerative colitis (UC), the same bacteria that are included in ProBiotic 3, it was found that administration of BIO-THREE improved the clinical symptoms and endoscopic findings in patients with UC, indicating that administration of BIO-THREE is safe and efficacious for the treatment of UC.
Twenty patients who presented with moderate distal UC took 9 tablets of the probiotic each day. Clinical symptoms and endoscopic findings were evaluated as ulcerative colitis disease before and after administration of BIO-THREE. Fecal samples were collected from all patients before and after probiotics administration, and fecal microflora was analyzed.
Remission was observed in 45% (9/20) of the patients. T-RFLP analysis indicated that the principal alteration in microflora was an increase in bifidobacteria.
Bacterial Vaginosis Treatment With Bio-three
In a study investigating the treatment of bacterial vaginosis and vaginitis with Bio-three, 16 females participants with bacterial vaginosis and vaginitis were instructed to use an intravaginal application of 2 g of Bio-three (E. faecalis T-110, C. butyricum TO-A, B. mesentericus TO-A, pH 6.9 +/- 0.3).
The results of the treatment were evaluated 3 days following administration of the Bio-three treatment by observing the vaginal discharge and completing an assessment of the bacteriological strains present in the discharge.
A clinical improvement was noted, and the amount of vaginal discharge and vaginal redness decrease were significant; the PH of the vagina was also lowered significantly. 35 bacterial strains were originally detected and present in the vaginal discharge, just 3 days after administration, only 16/30 Gram-positive bacterial strains, and 2 strains of Gram-negative bacteria were not present any longer.
In terms of the total bacteriological effects, 7/16 cases were eradicated, 1 case was partially eradicated, 6 cases were replaced. The results indicated that therapy with Bio-three is effective in both bacteriological and clinical responses.
Probiotic products on the market: the facts!
• All probiotic products are NOT alike and do not offer the stated health benefits.
• Over 99% of the products on the market have never been clinically studied and make unproven claims!
• Most formulas are a guess work in compilation of the greatest number of different strains in the highest number per capsule to impress the consumer e.g. 12 strains delivering 20 billion cfu (colony forming units). Do not be fooled with the concept of ‘more is better'. The reason most companies put in such large numbers is because they have no clinical data that any of the strains are compatible, survive or produce any effect and are hoping that by incorporating such high amounts at least some will end up surviving. Do not be fooled!
• Some combinations have no history of use and may be antagonistic to each other and, worse still, may alter the gut flora in an undesirable way.
There are many different types of probiotic supplements available on the market today. Most have multiple strains in the formulas and may not have been studied in combination with one another. The most common probiotic formulas contain a combination either acidophilus and or bifidobacteria as the main ingredient. Current recommendations to take a product containing at least 20 billion, and up to 120 billion organisms per capsule, are based on the fact that many strains have not been studied in combination, therefore many of the organisms may not survive together. This results in the philosophy that if we don't think many will survive, take as many as possible. But more is not always better!
